How to Register
Step 1 – Prepare Your Documents
Bring the following to Health Screen Laboratory and Diagnostic Center – Makati Branch:
- PhilHealth ID or Member Data Record (MDR)
- Valid government-issued ID
- Doctor’s prescription (if claiming medication)
Step 2 – Schedule Your Visit
Call us at 0939-888-5389 to book an appointment, ensuring that a doctor will be available for consultation.
Step 3 – Visit the Branch
Proceed to our front desk at Health Screen Makati where our staff will:
- Verify your eligibility
- Guide you through the registration process
- Facilitate your consultation with the doctor
Step 4 – Receive Your Medicines
- If you are seen by our Health Screen doctor, your prescription will be processed, and you can access medicines through the Gamot Pharmacy program under the PhilHealth YAKAP initiative.
- Patients already seen by a doctor at another Yakap facility may present their UPSC-coded prescription and valid ID at an accredited Gamot Pharmacy to receive their free medications.
